Business Office FAQ

  1. How much will my procedure cost?
  2. We are contracted with a number of insurance plan(s).  The insurance will process the procedure claim(s) based on the patient’s policy.  If you have a procedure at one of our Ambulatory Surgery Centers, there will be 3 to 4 claims submitted to your insurance plan(s): the physician, the facility, the anesthesia, and pathology (if a specimen was collected).  Cost of the procedure depends on patient’s policy with their insurance plan(s).  Each procedure is specific to that patient.  Refer to the Price Quotes for your procedure type.  Contact your insurance plan(s) to check your benefits.

  3. What is the difference between a Screening Colonoscopy and a Diagnostic Colonoscopy?
  4. There are 3 types of Colonoscopies: Screening, Diagnostic and Surveillance.  Please review the following definitions and call your insurance plan(s) to check benefits.

    1. Screening/Preventive – Patient is asymptomatic (no gastrointestinal symptoms either past or present), over the age of 50, has no personal or family history of GI disease, colon polyps, and/or cancer.  The last Colonoscopy was 10 years or greater.
    2. Diagnostic – Patient has past and/or present gastrointestinal symptoms, polyps, GI disease, iron deficiency anemia and/or any abnormal tests.
    3. Surveillance/High Risk Screening – Patient is asymptomatic (no gastrointestinal symptoms either past or present), has a personal history of GI disease, colon polyps, and/or cancer and family history of colon polyps, and/or cancer (with last Colonoscopy less than 10 years ago).  Surveillance/High Risk Screening Colonoscopies are recommended every 2 years or more.

  5. What is my co-pay?
  6. Co-pays are determined by your insurance plan(s) and could be listed on your insurance card.  If co-pays are not listed on your card, please contact your insurance plan(s).  Dayton Gastroenterology is considered a specialty physician, so the co-pay may be higher than your primary care physician.  Co-pays are due at time of service.  Dayton Gastroenterology does access a $20 fee if co-pays are not paid at time of service.

  7. Do you accept my insurance?
  8. Dayton Gastroenterology is contracted with a number of Insurance Plan(s).  This is not an inclusive list, as some are plans are priced by a larger plan.  If your insurance is not listed, please call your insurance and ask if Dayton Gastroenterology is in network. Our Tax ID # is 311070615.

  9. Why did I receive more than one statement for my procedure?
  10. If you have a procedure at one of our Ambulatory Surgery Centers, there will be 3 to 4 claims submitted to your insurance plan(s): the physician, the facility, the anesthesia, and pathology (if a specimen was collected).  The insurance plan(s) process these claims separately and are paid separately.  You could receive more than one statement for your procedure.

  11. Do you offer financial assistance?
  12. Dayton Gastroenterology offers various forms of assistance for patients.  Please view our Financial Assistance options.  Please contact the Business Office at 937-320-5055 if you have further questions.

  13. How can I pay my bill?

Dayton Gastroenterology accepts all forms of payments: cash, checks, money orders, and all major credit cards/debit cards. 

  • You can mail in your payment with your statement.
