Dayton Gastroenterology accepts Medicare, Medicaid, most commercial insurance carriers, and all major credit cards.
Important reminders to assist in your care:
- Co-payments are due at time of service
- Bring a Photo I.D.
- Bring insurance card(s)
- Bring a list of current medications and dosages
- Bring completed forms
- Patients are responsible for obtaining any referrals and knowing the requirements of their insurance carrier, including pre-certification
- Patients are financially responsible to Dayton Gastroenterology, Inc. for non-payment of any fees not covered by your insurance
- Account statements are mailed monthly
- Physician, facility, and lab charges are billed separately
- $20 service charge will be assessed if co-payment is not paid at the time of service
